Do I need a prescription for physical therapy from a doctor?
Due to recent legislation changes in New York State you no longer need a prescription for the Initial Evaluation and the first 10 visits or 30 days of physical therapy, whichever comes first. We will provide you with a Notice of Advice form that covers these visits without a prescription. Should you need to continue your physical therapy program for more than 10 visits or 30 days, it is necessary to have a prescription from your specialist or primary care physician
Do I need a referral from my primary care physician?
Some insurance companies require a referral, as well as a prescription from the doctor. An insurance referral is generated either from the primary care physician (PCP) or sometimes from your orthopedist. We will let you know if this is a requirement specific to your plan.
What should I bring on my first visit?
Please bring a comfortable change of clothing for the initial examination (t-shirt, shorts, sweatpants). Even if your injury is just a toe, observation of your knee, hip, pelvis, or your torso during gait may be relevant to how the injury occurred & what physical therapy may entail for complete recovery. Or if your diagnosis is carpal tunnel syndrome, we are very concerned with what is happening in your cervical and thoracic spine, your scapular positioning, development of your shoulder muscles, and especially the ergonomics of how you work at your desk. What is required for subsequent visits will be discussed during your initial visit.
We do have private locker room facilities where you can change, and private treatment rooms for any confidential history you share during the initial consultation.
Do I need forms and paper work for the first visit?
Unfortunately, yes. But to minimize the time sitting in our waiting room, we have made the forms available for you to download from this website. Please refer to the download the forms option to access what is necessary prior to the first visit.
Once the paperwork is completed, please fax completed forms to the number below.Please do not hesitate to call if you encounter any problems with downloading or faxing.
It is necessary for us to receive these forms prior to your first visit. This will allow optimal time and attention to your condition – and not the paperwork.

What time should I arrive for my first visit and subsequent visits?
We recommend that you arrive 10 minutes early for your initial consultation.
Subsequent visits are on the hour or half hour, and we need you to be changed and ready at your appointed time. We understand the value of your time, and do our best to keep on schedule. Depending on your injury, it may be advantageous for you to come early for a light aerobic workout, stretching, or to apply heat to a stiff joint prior to manual therapy. Your therapist will determine any pre-treatment requirements. Also, we encourage our patients to take advantage of our aerobic and strengthening equipment independently, when approved and indicated by the therapist.
Are all of your therapists licensed?
Park South Physical Therapy only employs New York State Licensed Physical Therapists.
